Appointment Information At Cardiovascular Associates, we believe it’s essential that
you and your doctor have a good rapport. You can make an appointment
for a complimentary initial interview with your physician to see
if our personal approach is right for you. To make an appointment,
call
985-871-0735. We see patients only by appointment. Since it's important to us
and to you that all appointments be kept on time, please notify
us of any cancellation as far in advance as possible. We respect your time and make every effort to stay on schedule.
Because our practice sometimes involves treating critically
ill patients and emergencies, though, we occasionally have delays.
Whenever possible,
we will notify you in advance of emergency delays. If you must contact your physician after hours for an urgent
medical reason, our answering service will forward your message.
(back to top) Your First Appointment
When you come to your first appointment, you’ll review your
medical history with one of our registered nurses before seeing a cardiologist.
Your doctor will then discuss all pertinent information with you.
It’s important that we have as much information as possible
regarding your cardiac history. At your first visit, please bring: Information about any prior heart disease, stenting or balloon angioplasty
- History of blockages in the arteries of the legs, kidneys or neck
- History of prior bypass surgery and location of bypass grafts
- All insurance information and cards
- A primary care referral if required by your insurer
- Written list of allergies
- Written list of current medications and dosages
- Written questions for your physician

Insurance and Financial Information Insurance and Managed Care
We accept most insurance
and participate in many managed care plans. We also accept Medicare
assignments.
While you are ultimately
responsible for payment of all charges for services
you receive, we will assist you in any way we can,
including filing insurance
claims for covered procedures. If your insurance requires that lab work be sent
to a reference laboratory, please let us know
in advance. Fees, Copays, Coinsurance and Deductibles
We will be glad to discuss any questions about
fees and payments with you. Feel free to contact
our billing
department at
985-871-0735. If your health care plan includes a copayment,
it is due at time of service. You can pay with
cash, check,
VISA or MasterCard. Patients are responsible
for co-insurance,
non-covered services
and annual deductibles.
If for any reason your insurer has not paid
within 60
days of
our claim, you may be held responsible for
full payment. Referral
While your primary care physician may refer
you to a specialist at Cardiovascular Associates,
it is not necessary to have
a referral in order to see one of our physicians
unless required by
your
healthcare coverage. In that case, you need
to
get a
referral before your appointment
with your Cardiovascular Associates physician.
